Physician Assistant/ Nurse Practitioner Opportunity

Job Summary:
We are seeking a highly skilled Physician Assistant or Nurse Practitioner to join our team as a House Call Health Assessment Specialist. As a member of our fast-growing team, you will have the opportunity to conduct health risk assessments on our patient population in the comfort of their own homes.
Key Responsibilities:
- Perform in-home health assessments of high-risk patients, including medication review, Point-of-Care testing, and physical examination.
- Identify opportunities for preventive care, health promotion, home safety, and illness prevention.
- Educate patients on health conditions, medications, and nutrition, and emphasize the importance of consistent monitoring of health needs with their Primary Care Physician (PCP).
- Communicate urgent medical needs and collaborate with the patient's PCP for clinical escalation and care management needs as needed.
Requirements:
- Physician Assistant with a BS/MA Degree from an ARC-PA accredited Physician Assistant Program or a Master level or above degree from an accredited Nurse Practitioner program.
- Current New York State Physician Assistant or Nurse Practitioner License.
- BLS certification.
- New graduates are welcome, 1+ year(s) experience in Family Practice/Internal Medicine/Geriatrics preferred.
- Excellent customer service skills and ability to travel.
- Bilingual in English and Chinese (Mandarin or Cantonese) required.
Compensation:
$100-$125 per case with transportation bonus.
