The University of Maryland Medical System is seeking a highly skilled and experienced Population Health Program Manager to oversee the development and implementation of care management programs. This individual will be responsible for designing workflows to meet program goals and metrics, maintaining systems to ensure compliance with accrediting and professional certification bodies, and collaborating with primary care practices, specialty clinics, and other healthcare organizations.
Key Responsibilities- Develop and implement care management programs to improve population health outcomes and maximize value-based care initiatives.
- Support innovation in clinical practice and healthcare delivery systems by disseminating research and innovation into practice.
- Create management systems that promote standardization of workflows, assure achievement of process goals and quality metrics, and incorporate all applicable NCQA accreditation standards.
- Facilitate the development, implementation, and expansion of the care management program, including the Quality Care Network and Transform Health MD initiatives.
- Manage the effective use of patient registries to manage specific patient populations and improve disease and population health outcome measures.
- Collaborate with other departments and colleagues across the continuum to address gaps and improve care coordination.
- Comply with federal, state, and local legislation and accrediting bodies that may impact the organization's future goals and objectives.
- Develop and foster strong relationships with Quality and Compliance, and serve as a primary lead for Care Management for NCQA activities.
- Collaborate with other departments, including Clinical Integration, Data, Quality, and Practice Transformation, to identify data needs related to care management and implement performance/outcome measures.
- Facilitate and support an interdisciplinary practice environment that promotes empowerment, responsibility, autonomy, accountability, collaboration, and peer review.
- Effectively articulate the relationship between strategic goals for patient satisfaction, clinical and quality outcomes, and financial performance.
- Implement systems to manage and improve the performance, consistency, and integration of care coordination and care management services.
- Demonstrated working knowledge of Care Management principles and NCQA accreditation requirements.
- Demonstrated working knowledge of Population Health incentives, including Value-Based Care principles, HEDIS metrics, quality outcome data, reimbursement models, and HSCRC initiatives.
- Demonstrated success in a leadership role, exhibiting teaching, coaching, and development skills, and effectiveness in a consultative role at the organizational level.
- Ability to supervise, coach, and create a motivating environment for clinical and professional personnel.
- Highly effective interpersonal, verbal, and written communication and presentation skills.
- Ability to analyze and conceptualize data to ensure achievement of desired outcomes.
