Job Title: Hematopoietic Transplant Coordinator
Job Summary:
We are seeking a highly skilled and compassionate Hematopoietic Transplant Coordinator to join our team in a specialized patient care environment. The coordinator will be responsible for managing all aspects of patient care related to hematopoietic stem cell transplantation, including pre-transplant evaluation, donor matching, and post-transplant follow-up.
Key Responsibilities:
- Coordinate the entire hematopoietic transplant process from patient referral, evaluation, donor matching, through to post-transplant follow-up.
- Serve as a liaison between patients, families, physicians, and the transplant team to ensure clear communication and coordination of care.
- Educate patients and their families on the transplant process, including risks, benefits, and post-transplant care expectations.
- Oversee the scheduling of diagnostic tests, transplant procedures, and follow-up appointments.
- Collaborate with a multidisciplinary team, including hematologists, oncologists, transplant physicians, and nurses, to develop individualized patient care plans.
- Coordinate donor search and selection in conjunction with national and international registries.
- Monitor patient progress before and after transplant, managing complications and adjusting care plans as needed.
- Maintain detailed and accurate records of transplant procedures, donor information, and patient outcomes in electronic health records (EHR).
- Ensure adherence to clinical protocols and regulatory guidelines related to transplant procedures.
Qualifications:
- Active Registered Nurse (RN) license in the state of Florida.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 3 years of experience in hematology, oncology, or transplant nursing.
- Experience with stem cell transplantation or cellular therapy highly preferred.
- Excellent communication, organizational, and leadership skills.
- Ability to work collaboratively in a multidisciplinary team environment.
- Proficiency in using electronic health records (EHR).
