We are seeking a skilled Licensed Practical Nurse to join our continuing care team, providing high-quality care to our residents.

What we offer:

Compensation:
Ranging from $32.00 per hour to 39.00 per hour, based on years of licensure, with potential for shift differential.

How you will make an impact:

Developing relationships with residents and their families, using a consistent caregiver approach to support their physical, spiritual, emotional, and clinical needs. Assessing and evaluating resident clinical conditions, documenting findings, and notifying providers as needed. Updating care plans to reflect changes in resident care and service needs. Supervising and leading care associates, administering medications, and participating in meetings related to resident outcomes and care.

Responding to residents, family, or responsible parties in a timely and professional manner. Participating in the review of clinical indicator reviews, such as falls, wounds, nutrition, and antipsychotic use. Other duties as assigned by RN clinical leader.


What you will need:
A minimum of 1 year of experience working as an LPN, with experience in assisted living, home health, or long-term care preferred. A passion for caring for seniors and being a strong resident advocate.
