Registered Nurse, Admissions Specialist

Chapters Health System is seeking a highly skilled Registered Nurse to join our Admissions team. As a key member of our hospice care team, you will provide exceptional patient care and support to patients and their families in a collaborative and compassionate environment.
Responsibilities- Conduct initial nursing assessments, develop care plans, and provide ongoing care to patients in accordance with public health nursing standards and policies.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Supervise and mentor LPNs, CNAs, Unit Secretaries, and Volunteers to ensure high-quality patient care.
- Develop and maintain strong relationships with physicians, payers, patients, and co-workers to ensure effective care delivery.
- Stay up-to-date with industry developments, best practices, and regulatory requirements to ensure compliance and excellence in care delivery.
- Minimum of two years of public health/community health, med surg, or related nursing experience.
- Experience in home care, hospice, and/or oncology nursing preferred.
- Diploma or Associates degree in Nursing from an accredited nursing program; BSN preferred.
- Licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice.
- Exceptional oral and written communication skills.
- Ability to organize and manage projects within defined timeframe and budget.
- Strong interpersonal and collaborative skills.
- Ability to work independently and as part of a team.
- Excellent problem-solving and conflict resolution skills.
Works in a business office with computers, printers, copy/fax machines, and telephones. The noise level in the work environment is usually light to moderate.
