POSITION SUMMARY
The RN Case Manager acts as a patient advocate, guiding and coordinating care for patients, families, and caregivers throughout their healthcare journeys. This role involves collaborating in the development of personalized care plans, ensuring efficient resource utilization, identifying patient needs, and establishing safe discharge plans.
KEY RESPONSIBILITIES
- Conduct comprehensive assessments of patients to identify clinical, psychosocial, and financial factors affecting care progression.
- Evaluate patient and family understanding and engagement in care goals, integrating insights into the care plan.
- Coordinate services among community agencies, healthcare providers, and families involved in the care process.
- Provide patients and families with essential tools and resources to facilitate informed decision-making.
- Develop clinically appropriate care plans that align with patient needs and available resources.
- Ensure effective communication during transitions of care, involving all relevant stakeholders.
- Identify community resources and advocate for addressing gaps in care availability.
- Maintain compliance with federal, state, and local regulations impacting patient advocacy.
- Participate in multidisciplinary rounds to ensure appropriate patient status and care level.
- Document interventions to support timely discharge and address any medical necessity issues.
- Educate patients and families on the financial implications of care options.
- Monitor resource utilization and address any patterns of avoidable delays.
- Support the appeals process for denied care by providing necessary clinical information.
- Engage in performance improvement initiatives to enhance care delivery.
- Collaborate with the utilization review team to meet organizational goals.
- Recognize and refer situations requiring quality or risk management intervention.
QUALIFICATIONS
- Associate Degree in Nursing required; BSN preferred.
- Minimum of 1 year of experience in discharge planning, case management, or as a Registered Nurse in a healthcare setting.
- Current Registered Nurse (RN) license in the state of Florida.
- Basic Life Support (BLS) certification from an accredited organization.
- Preferred Case Management Certification.
