Care Planning Specialist

Lanessa Extended Care in Webster, Connecticut, is seeking a skilled Care Planning Specialist to join its dynamic care team. This role is perfect for individuals who are passionate about delivering high-quality care and services to residents in a long-term care setting.
Competitive Salary: $38/hour for LPNs and $45/hour for RNs, paid weekly.
The Care Planning Specialist will play a crucial role in assessing and documenting residents' needs, developing care plans, and ensuring compliance with regulations. This position requires prior MDS experience and a strong understanding of nursing and medical practices, as well as laws, regulations, and guidelines that pertain to long-term care.
Key Responsibilities:
- Coordinate the MDS process, including overseeing ADL training for facility and staff.
- Ensure timely and accurate completion and transmission of all CMS-approved item sets (MDS).
- Maintain knowledge of and follow current CMS regulatory guidelines as described in the RAI Manual.
- Protect residents' individual health information and maintain adequate systems to ensure appropriate documents are sufficient to support billed services.
Requirements:
- A minimum of a Nursing Degree from an accredited school of nursing, college, or university.
- A current, unencumbered license to practice as an RN or LPN in the state of Connecticut.
- Prior MDS experience and strong leadership and supervisory abilities.
