Job Summary:
In this pivotal role, you will provide high-quality patient care and coordinate clinical operations to ensure seamless delivery of infusion services.

Your Role:
You will work under the supervision of a designated supervisor and be responsible for coordinating and providing intermediate direct patient care to patients in their homes or alternate infusion suites. This includes developing and implementing nursing plans of care, assessing patient needs, and communicating effectively with healthcare teams to achieve established goals.

Responsibilities:
- Develop and implement comprehensive nursing plans of care for patients, ensuring continuity and compliance with physician orders
- Assess patient needs through health data access and patient interviews, re-evaluating progress towards goals as needed
- Educate patients and families/caregivers on prescribed therapies and care needs using a patient-centered approach
- Collaborate with multidisciplinary teams to facilitate effective communication and coordination of care
- Participate in training and mentorship activities to support the development of Infusion Nurses I and other team members

Requirements:
- Active and unrestricted RN license in the state of practice
- Minimum 1 year of nursing experience
- Current CPR certification required
- Demonstrated competency in patient care standards and infusion skill sets applicable to agency programs and service needs

Salary:
The estimated salary range for this position is $48.45-$79.14 per hour, commensurate with your education, experience, and qualifications.
