Job Description

We are seeking a skilled Nurse Navigator to join our inpatient Supportive and Palliative Care team at ChristianaCare in Newark, DE.

About the Role

In this critical position, you will play a vital role in navigating and facilitating the care of patients and their families seeking palliative care. Collaborating with the multi-disciplinary care team, you will assist in coordinating referrals, completing patient reviews, and identifying potential barriers to care.

Responsibilities
  • Coordinate referrals from within and outside of Christiana Health Care System.
  • Complete patient reviews upon initial encounter and periodically throughout navigation.
  • Assist with identifying potential and realized barriers to care and facilitate referrals.
  • Schedule and/or facilitate scheduling of appointments.
  • Triage family/caregiver phone calls and messages, determining disposition and communicating treatment plans to patients/caregivers.
  • Organize inpatient palliative census and daily workload, evaluating and triaging consults with the hospital team.
Requirements

To be successful in this role, you must have:

  • A BSN degree, with a Master's in nursing preferred.
  • RN license or eligibility for licensure in the state of Delaware.
  • Three or more years of combined clinical experience in case management, clinical trials, older adult, palliative/hospice care.
  • Nursing Cert - Certified Hospice and Palliative Nurse (CHPN) within 3 years of hire.
Salary

The estimated annual salary for this position is $83,000 - $110,000, based on national averages and location factors.
