Nurse Care Coordinator

We are seeking a highly skilled and compassionate Registered Nurse to join our team at New Transitions, Inc. as a Nurse Care Coordinator. In this role, you will be responsible for creating and managing personalized care plans for individual patients.
Key Responsibilities:- Care Plan Development: Develop and implement standardized procedures for medical record keeping, ensuring compliance with all regulatory requirements and best practices.
- Staff Training and Development: Instruct staff on nursing best practices, client care standards, and safety protocols, and develop and conduct training programs and in-services for new and existing staff.
- Client Care and Supervision: Conduct comprehensive client assessments and develop individualized care plans, oversee the administration of medications and treatments, monitor client progress, and provide direct client care as required.
- Quality Improvement: Identify areas for improvement in client care and operational efficiency, participate in quality improvement initiatives, and implement changes as needed.
- Currently licensed Registered Nurse (RN) in Minnesota.
- Previous experience as a Registered Nurse is preferred.
- Excellent interpersonal and client care skills.
- Valid driver's license.
- First aid/CPR certification required.
Salary: $80,000 - $110,000 per year, depending on experience.
