Bilingual Manager, Complex Care Coordination Specialist

Homeland LLC is seeking a highly skilled Bilingual Manager, Complex Care Coordination to join their team. As a key member of the organization, you will play a pivotal role in enhancing the care experience for members with complex dental needs.
The ideal candidate will possess excellent leadership and team management skills, with the ability to motivate and inspire team members. They will also have a strong understanding of dental health issues, Medicaid regulations, and interdisciplinary healthcare delivery.
Responsibilities
- Develop and implement comprehensive care coordination processes to ensure seamless support for members
- Assess individual needs and create tailored care plans to address complex dental issues
- Collaborate with healthcare providers to improve health outcomes and member satisfaction
- Lead a team of care coordinators to ensure compliance with state and federal regulations
- Develop and implement training programs to foster a culture of continuous improvement
- Excellent verbal and written communication skills to effectively interact with members, providers, and team members
- Ability to convey complex information clearly and empathetically
- Strong analytical and critical thinking skills to assess complex situations and develop effective solutions
Requirements
- Bachelor's degree in Dental Hygiene or a related field
- 5+ years of experience in healthcare management, care coordination, or case management, preferably with a focus on dental services
- Proven ability to lead high-performing teams in a metric-driven environment
- Track record of successfully meeting key performance metrics and service level agreements on a monthly and/or quarterly basis
- Demonstrated focus on coaching and developing team members, ideally through a structured coaching and quality program
- Strong skills in Microsoft Excel, including the ability to create and analyze pivot tables, charts, and basic formulas for data reporting and analysis
- Proficient in Microsoft Office tools, such as Word and PowerPoint, for documentation and presentations
- Ability to maintain confidentiality and adhere to HIPAA requirements
- Excellent verbal and written communication skills, with the ability to document and present information empathetically and clearly
- Must be adaptable, flexible, and able to adjust to changing situations with diverse teams
- Highly organized, detail-oriented, and a self-starter with strong analytical skills and the ability to multitask
- Strong knowledge of Medicaid policies and procedures related to dental care, preferred
- Certification in Care Coordination or Case Management (e.g., CCM, CMC) is a plus
