Key Responsibilities:
Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders
Develop and implement individualized plans of care that meet the unique needs of each patient
Utilize standardized assessments and evidence-based practice to support clinical interventions
Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders
Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, and modalities
Assess patient progress and adjust treatments accordingly
Evaluate treatment outcomes and modify services as needed
Provide consultation and counseling to patients, families, caregivers, and other service providers
Effectively communicate with supervisors and other healthcare team members regarding patient progress, barriers, and treatment plans
Provide comprehensive discharge summaries of services provided
Qualifications:
Licensed Occupational Therapist
This position is open to OTs at all experience levels
