Job Summary:

The successful candidate will utilize the nursing process to assess, implement, coordinate, and evaluate patient care activities. This will involve collaborating with other clinical associates and healthcare disciplines to meet individual needs using appropriate resources to ensure quality, safe, and cost-effective care.

Nurturing a working relationship with physicians, nursing staff, and other disciplines involved in the care of the patient/family is essential for this role. The ideal candidate will be able to provide exceptional care and support to patients and their families.

Responsibilities:

  • Assess patient needs and develop individualized care plans
  • Implement and coordinate patient care activities
  • Evaluate patient outcomes and make adjustments as needed
  • Collaborate with multidisciplinary teams to achieve quality patient care
  • Maintain accurate records and communicate effectively with patients and staff

Requirements:

  • Degree in Nursing (RN)
  • Licensed to practice as a Registered Nurse in [State]
