Clinical Care Coordinator RN

We are seeking a registered nurse to coordinate nursing care for patients in our Physician Clinics.
Job Summary:The ideal candidate will possess strong work ethics, critical thinking skills, and excellent communication skills. The RN will be responsible for coordinating nursing care, participating in patient and family teaching, and providing leadership in the daily operations of our clinics.
About the Role:- Assess patients systematically and plan appropriate nursing care.
- Implement care based on knowledge of professional standards of nursing practice.
- Collaborate with physicians to develop and implement patient care plans.
- Identify learning needs of patients and family members and provide education through the teaching/learning process.
This is a float position that requires travel to all 5 campuses and outlying clinics. The office hours are Monday through Friday, 8:00 am to 5:00 pm, with no requirement to work weekends or holidays.
Requirements:- A current license to practice as a professional nurse in the state of Louisiana.
- Department orientation will be provided based on experience and level of comfort.
- Hospital-wide orientation required upon hire.
$65,000 - $85,000 per year, depending on experience and qualifications.
