Occupational Therapist

We are seeking a skilled Occupational Therapist to join our Home Health team. As a key member of our care team, you will work closely with patients to develop and implement personalized rehabilitation plans, helping them to regain their independence and improve their quality of life.
Responsibilities- Assess patients' daily living and work-related skills, and develop therapeutic retraining programs with measurable objectives.
- Administer and interpret diagnostic and prognostic tests to assist physicians in evaluating patients' level of function.
- Confer with physicians and other healthcare team members to develop and revise treatment plans.
- Provide therapeutic treatment and instruction to patients, improving their strength, coordination, range-of-motion, and function, or teaching compensation measures.
- Review and expand retraining programs to align with patients' behavioral goals and rehabilitation program guidelines.
- Consult with vocational team members to develop and implement vocationally oriented plans.
- Report patient status and progress to physicians, rehabilitation staff, Clinical Managers, patients, and family members.
- Participate in care coordination and discharge planning activities, and serve as a resource to other healthcare personnel.
- Design community reintegration activities to assist clients in physical reconditioning and psychological adjustment.
- Recommend and design special adaptive equipment to improve patients' residence or working environments.
- Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills.
- Coordinate and oversee Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
- Accurately document patients' care observations, interventions, and evaluations.
- Degree from an accredited Occupational Therapy Program.
- A minimum of six months of occupational therapy experience, with Home Health experience preferred.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
