About the Role

As a Home Health Occupational Therapist Assistant with CenterWell Home Health, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes. Our team is dedicated to helping patients manage chronic conditions, recover from illness or injury, and regain their independence.

Key Responsibilities
  • Provide therapy services as planned and delegated by the qualified Occupational Therapist, in accordance with the patient's Plan of Treatment.
  • Assist in the implementation of vocational/education programs and activities designed to restore, reinforce, and enhance task performances.
  • Design and adapt equipment and working environments to promote patient independence and participation in daily activities.
  • Provide therapeutic treatment and instruction to patients as directed by the qualified Occupational Therapist, to improve strength, coordination, range-of-motion, and function.
  • Report information and observations to the Occupational Therapist and/or Clinical supervisor, and document patient records and clinical notes.
  • Participate in care coordination and discharge planning activities, and act as a resource to other healthcare personnel in meeting patient needs.
Requirements
  • Current and unrestricted OTA licensure.
  • Minimum of six months occupational therapist assistant experience preferred.
  • Home Health experience a plus.
  • Current CPR certification.
  • Good organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
