Participant Care Associate

POSITION: PARTICIPANT CARE ASSOCIATE (PCA)
The Participant Care Associate (PCA) plays a vital role in delivering essential support services to participants, ensuring their comfort and well-being in various settings, including home, community, and health centers. Under the guidance of the Center Manager or Team Lead, the PCA works closely with registered nurses to implement individualized care plans tailored to each participant's needs.
KEY RESPONSIBILITIES:
- Develop and follow a personalized care plan based on comprehensive assessments conducted by a registered nurse case manager.
- Assist in providing physical and emotional support to elderly participants, fostering independence and self-reliance.
- Deliver a range of services as outlined in the participant's Care Plan, including:
- Personal hygiene assistance
- Support with daily activities
- Meal preparation and feeding assistance
- Toileting support and incontinence management
- Basic medical treatments
- Shopping and errands
- Medication reminders
- Laundry and housekeeping tasks
- Transportation to appointments
- Maintain accurate and timely documentation of care provided during each visit, ensuring effective communication with nursing staff regarding participant progress.
- Report any service-related issues or schedule deviations to the supervisor promptly.
- Respond to emergencies following established protocols.
- Adhere strictly to the defined scope of practice.
WORKING CONDITIONS:
- Variable working conditions with potential exposure to communicable diseases and cleaning chemicals.
- Monthly supervision by a registered nurse case manager to evaluate participant satisfaction and PCA performance.
- Driving is required within the designated service area, necessitating reliable transportation and a valid driver's license.
- Physical demands include the ability to assist with patient mobility and perform tasks that may involve lifting up to 40 pounds.
- Flexibility in participant assignments may be necessary based on individual needs and acuity levels.
QUALIFICATIONS:
- High School diploma or GED is required.
- Valid driver's license and current CPR certification are mandatory.
- Preferred candidates will have certification in a relevant field and at least one year of experience working with elderly or frail populations.
- Completion of a formal training program in direct care or a related area is essential.
- Participation in ongoing training and competency assessments is required to maintain skills and knowledge.
- Strong interpersonal skills and the ability to work effectively with diverse populations are essential.
- Ability to manage multiple tasks and adapt to changing circumstances is necessary.
- Medical clearance for communicable diseases and up-to-date immunizations are required prior to engaging in direct participant contact.
