Job Summary

We are seeking a highly motivated and compassionate Case Manager/Assistant Youth Coordinator to join our team at Axess Family Services. The successful candidate will provide case management services to individuals and families, as well as support and advocacy for participating youth.

Key Responsibilities
  • Implement or coordinate supportive services that foster social, emotional, and educational growth.
  • Conduct assessments and provide age-appropriate case management.
  • Provide referrals and assist families in their interactions with schools, healthcare providers, and social services as needed.
  • Report outcome measures and demographics as required.
  • Maintain confidential client records, including up-to-date data entry as required by the funder.
  • Maintain relationships with community social agencies to capitalize on resources on behalf of the Center and clients in need of assistance.
  • Provide a supportive, trauma-informed environment to families.
  • Recruit and train volunteer tutors during the school year for grades K-3 and manage classroom behaviors.
  • Pair students and tutors for optimal learning.
  • Communicate with Kenston School District's administration and teachers concerning student progress.
  • Organize and maintain library.
  • Participate in team planning and program goal achievements.
  • Use Microsoft Office functions, email, calendar, and other data methods needed for documentation.
  • Attend meetings and trainings as assigned.
  • Comply with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.
Requirements
  • Candidate must have knowledge and ability to establish client relationships with professional boundaries.
  • Willingness to learn client-centered model and conflict resolution; ability to manage crisis situations.
  • Ability and understanding of case management record keeping, ability to communicate with clients and build trust, strong overall communication skills, both oral and written.
  • Ability to model open communication, fair and equitable treatment, and conflict management skills.
  • Knowledge and ability to use Microsoft Office suite for recording information.
  • Mandatory CPR and First Aid Certifications.
  • Valid Ohio Driver's License and ability to meet agency requirements for driving insurability.
  • Must be able to meet agency requirements for criminal background check (Ohio Bureau of Criminal Identification & Investigation).
Education and Experience

Minimum educational/licensure requirements: Degree in Social Work, Counseling, Psychology, Human Services, or related degree preferred. High school diploma/GED required.

Minimum experience requirements: Experience working with populations that are low income and work with children in educational settings preferred.
