Hospice RN Case Manager Job Description

We are seeking a compassionate and dedicated RN to join our team as a Hospice RN Case Manager. In this role, you will provide exceptional end-of-life care, managing pain and symptoms, coordinating with interdisciplinary teams, and delivering holistic care tailored to individual needs.

Key Responsibilities:
  • Provide home-based care to patients with six months or less to live, focused on quality of life
  • Work as part of an interdisciplinary team to carry out the patient's plan of care
  • Perform patient assessments, administer medication and treatments, and ensure accessibility of medical equipment
  • Educate family on the patient's prognosis, changes in care plan, and hospice goals
  • Take a holistic approach to care that engages the patient's mind, body, spirit, and family
Requirements:
  • Graduate of an accredited nursing program and active RN license
  • Minimum of 1 year of hospice nursing experience
  • CPR certification
  • Valid driver's license, auto insurance, and reliable transportation
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing specialty, strong communication skills, and compassion
