We are seeking a dedicated and empathetic Registered Nurse MDS Specialist to join our team at Care Navigators On Demand, focusing on providing exceptional care in a skilled nursing environment.
Key Responsibilities:
- Under the guidance of the Director of Nursing Services, the Medicare/MDS Specialist is tasked with notifying and coordinating the Interdisciplinary Team (IDT) for the completion of MDS assessments in compliance with State and Federal regulations.
- Facilitates the Medicare/MDS resident assessment process.
- Ensures timely completion of the MDS Assessment by the Interdisciplinary Team.
- Oversees the development, implementation, and evaluation of individualized care plans.
- Administers or implements necessary treatments, medications, or nursing interventions as outlined in the resident's care plan or as prescribed by the physician.
- Provides nursing care in accordance with infection control protocols.
- Adheres to safety policies while delivering nursing care.
- Initiates emergency measures as required, following center policies and nursing standards.
- Demonstrated experience as a registered nurse.
- Minimum of one year of experience in a post-acute or long-term care setting is preferred.
- Ability to maintain composure in high-pressure situations.
- Compassionate and responsible demeanor.
- Strong interpersonal communication skills.
- Bilingual abilities are advantageous.
- AM shift availability.
- Consistent schedule from Monday to Friday.
