MDS Nurse

We are seeking a highly skilled and experienced MDS Nurse to join our team at Holiday Manor Care Center. As a MDS Nurse, you will play a critical role in ensuring the highest quality of care for our residents.
Key Responsibilities:
- Perform assessments of new patients and transmit MDS report forms for billing purposes.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Ensure accurate and timely completion of MDS reports and other documentation.
- Provide exceptional patient care and support to residents and their families.
Requirements:
- Current RN or LVN licensure in good standing.
- Minimum 2 years of experience in a Skilled Nursing Facility.
- MDS Certification preferred.
- Excellent communication, interviewing, and organizational skills.
