Key Responsibilities:
- Assess program participants with disabilities who are seeking services.
- Evaluate various factors including personality, vocational skills, and environmental influences.
- Maintain communication with referral sources regarding eligibility outcomes.
- Manage caseloads effectively, ensuring timely referrals to appropriate programs.
- Provide counseling to clients, addressing their emotional and disability-related concerns.
- Develop individualized rehabilitation plans based on assessment results.
- Prepare necessary documentation and secure approvals for rehabilitation services.
- Implement rehabilitation programs and monitor client progress.
- Coordinate employment placement services and evaluate job suitability for clients.
- Conduct follow-ups with clients to assess job satisfaction and service needs.
- Collaborate with OOD Liaison to ensure efficient eligibility determinations and case management.
- Ensure compliance with agency policies and maintain a safe working environment.
Qualifications:
- A Master's degree in Rehabilitation Counseling or a related field.
- Current certification as a Certified Rehabilitation Counselor (CRC) is preferred.
- Minimum of three years of counseling experience with individuals with disabilities.
- Knowledge of human services, rehabilitation principles, and community resources.
