Key Responsibilities

  • Assess and Develop Care Plans: Conduct thorough assessments to identify patient needs and develop personalized care plans in collaboration with the interdisciplinary team.
  • Manage Medications and Treatments: Administer medications and treatments as prescribed by the physician, ensuring accurate and timely delivery.
  • Provide Emotional Support: Offer emotional support and guidance to patients and their families, promoting comfort and dignity throughout the care process.
  • Educate and Collaborate: Educate patients and families about hospice care and end-of-life issues, and collaborate effectively with the interdisciplinary team to ensure coordinated care.
  • Document Patient Records: Maintain accurate and up-to-date patient records, ensuring seamless communication and continuity of care.
Requirements
  • Current RN License: Possess a current and active RN license in the state of employment.
  • Education and Experience: Hold a Bachelor's degree in nursing (BSN) and have a minimum of 1 year of experience in hospice care or end-of-life care settings.
  • Skills and Qualities: Demonstrate exceptional communication and interpersonal skills, with the ability to work independently and as part of a team. Possess a compassionate and empathetic approach to patient care.
