Hospice Nurse Job Summary

As a Hospice Nurse with Cantex, you will provide compassionate and high-quality care to patients and their families in their place of residence. Your primary functions will include administering nursing care, coordinating care with the interdisciplinary team, and assuming responsibility for the coordination of care.

Key Responsibilities:

  • Conduct initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs.
  • Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient eligibility.
  • Implement and document the plan of care to ensure quality and continuity of care, recommending revisions as necessary.
  • Consult with and educate patients and families regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
  • Determine scope and frequency of services needed based on acuity and patient/family needs.
  • Initiate preventive and rehabilitative nursing procedures.
  • Apply concepts of infection control and standard precautions in coordinating and performing patient care activities.
  • Provide clinical directions to Hospice Aides and LPNs/LVNs to ensure quality and continuity of service.
  • Prepare clinical and progress notes that demonstrate progress toward established goals.

Requirements:

  • Registered Nurse, licensed per state requirements.
  • 2 years of experience with a Hospice agency preferred.
  • Home Care Home Base experience preferred.
  • Good communication skills, assessment skills, and strong clinical documentation skills.
  • Ability to read, write, and speak English.
  • Reliable transportation and valid auto liability insurance.
