Position Information

The Care Coordinator is a Registered Nurse who works in a specific clinical care area, facilitating the plan for inpatient/outpatient care through a collaborative interdisciplinary approach.

The Care Coordinator focuses on anticipating, integrating, and coordinating clinical care and discharge planning services to meet the needs of patients and optimize clinical and resource outcomes.

The Care Coordinator collaborates with the interdisciplinary team to assess patient needs and develop the plan of care.

The Care Coordinator also acts as an advocate for the patient, physician, and hospital relative to compliance with federal, state, and third-party utilization management regulatory requirements.

Experience Requirements

3 years of experience in a nursing-related area is required, with clinical case management, clinical utilization management, or clinical home or community health experience preferred.

Education Requirements

A graduate degree in nursing is required, with a Bachelor's degree in nursing preferred.Licensure/Certification Requirements

A Registered Nurse license is required.

Hours of Work

Monday, Tuesday, Wednesday, 2pm to 10pm.

Weekend Requirements

No weekends.

Call Requirements

None.
