About the Role

We are seeking a skilled and compassionate Nurse to join our team as a Home Based Primary Care (HBPC) Outpatient RN. In this role, you will have the opportunity to make a meaningful difference in the lives of our nation's heroes by providing exceptional care and support to veterans in their homes.

Key Responsibilities

  • Deliver high-quality patient care to veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care.">
  • Assess, plan, implement, and evaluate care based on age-specific components, while utilizing interventions that maximize health capabilities and reduce unnecessary urgent care and emergency room visits.">
  • Collaborate with the interdisciplinary team to influence care outcomes and execute position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.">

Requirements and Qualifications

  • ">
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).">
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.">
  • Experience as an RN, with a minimum of 2 years Med/Surg experience, Home Health experience, Interdisciplinary Team experience, and American Heart Association BLS certification.">

Salary and Benefits

The salary range for this position is $91,854 - $154,434 per year, depending on education and length of practice. In addition to a competitive salary, we offer a comprehensive benefits package, including paid time off, retirement plans, and opportunities for professional growth and development.
