Job Summary
The Salvation Army is seeking a skilled Case Manager to join our team in a residential program for homeless individuals. As a key program role, the Case Manager will be responsible for evaluating the ongoing needs of program participants and providing intensive case management services to aid in their stabilization and minimize future episodes of homelessness.
Key Responsibilities
- Assess the individual's needs and develop case plans to include goals and objectives.
- Make referrals to other agencies and services as necessary.
- Provide follow-up to ensure that referrals are completed and track progress of residents.
- Facilitate communication and coordination of services between care providers.
- Promote resident self-advocacy and self-determination.
Requirements
- Bachelor's degree in a behavioral science-related field, such as social work, sociology, or psychology.
- Minimum two years of experience working in a residential program for homeless families.
- Minimum two years of case management experience working with disadvantaged individuals.
- Bilingual English/Spanish preferred.
- Ability to provide case management services, including resources, tools, and counseling to assist residents in achieving their case plan goals.
- Strong organizational and time management skills.
- Strong oral and written English communication skills.
- Ability to work collaboratively with other staff members, service providers, and professionals.
Working Conditions
The Case Manager will work in a fast-paced residential program environment, interacting with program participants, staff, and community partners. The ability to walk, stand, bend, squat, climb, kneel, and twist on an intermittent or sometimes continuous basis is required. The ability to lift 40 lbs and operate a computer, fax, and telephone is also required.
