Job Summary

We are seeking a skilled Behavioral Health Nurse RN to join our team as a Home Health Care Specialist. As a member of our team, you will have the opportunity to work with patients in their own homes, providing personalized care and support to help them achieve their health goals.

Responsibilities
  • Provide high-quality patient care and support to patients in their own homes
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Provide education and support to patients and their families to promote healthy behaviors and self-care
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
  • One year of recent nursing experience, recommended to be within the last 3 years, in an acute treatment unit in a psychiatric hospital, psychiatric home care, a psychiatric Partial Hospitalization Program (PHP), or another out-patient psychiatric service
  • Required Certifications and Licensures: Licensed to practice as a registered nurse in the state of agency operation, Must possess and maintain valid CPR certification while employed in a clinical role, Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
