Job Summary:Our Social Worker provides social work assistance to patients and families, addressing social determinants of health.Key Responsibilities and Qualifications:
  • Interview patients, assess priorities, and document case activity.
  • Consult with multidisciplinary team members to support patient treatment, leveraging community resources.
  • Assist patients/families in accessing community assistance by referring them to relevant resources.
  • Maintain working relationships with community health, welfare, and social agencies to aid patients in need.
  • Participate actively with the team to identify opportunities for improvement in standard work processes.
  • Remain vigilant and work safely at all times.
  • Perform other duties as assigned.
Requirements:
  • A Bachelor's degree in Social Work is required for this role.
Working Conditions:
  • Frequent reaching, sitting, standing, and walking.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more).
  • Frequent use of hearing and speech to communicate effectively through oral communication.
  • Frequent keyboard use/data entry.
  • Occasional bending, stooping, climbing, twisting, and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Occasional use of vision to identify and distinguish colors.
  • Occasional driving.
  • Rare repetitive foot/leg and hand/arm movements and squatting.
