About the Role

We are seeking a skilled Registered Nurse to join our Home Health Care team at Middlesex Health. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with Behavioral Health needs in the comfort of their own homes.

Key Responsibilities
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Provide acute psychotherapeutic and supportive interventions to patients and caregivers.
  • Collaborate with an interdisciplinary team, including providers, to ensure comprehensive care and achieve optimal patient outcomes.
  • Oversee the plan of care for approximately 25-30 patients within Middlesex County, managing acute care needs, including wound care, chronic disease management, and more.
  • Coordinate care alongside the interdisciplinary team under physician orders to ensure best outcomes for patients.
Requirements
  • Graduate of an accredited School of Nursing.
  • Possess the knowledge and skills necessary to assess health needs of patients and perform nursing care in a safe, caring manner.
