Recovery Coach

We are seeking a highly skilled and compassionate Recovery Coach to join our team at La Frontera Center Inc. As a Recovery Coach, you will play a vital role in providing services to children and their families, assisting in developing and implementing treatment plans, and coordinating recovery services.
Key Responsibilities- Establish a collaborative and respectful relationship with individuals and their families, focusing on outcome-driven goals.
- Collect and review intake and assessment documents, and other relevant materials to inform treatment plans.
- Develop and implement individualized service plans, taking into account the strengths, needs, preferences, and abilities of each individual and their family.
- Coordinate and implement services in accordance with state rules and regulations, ensuring the provision of necessary support and resources.
- Assist in maintaining and coordinating clinical records, ensuring accuracy and completeness.
- Provide services through face-to-face contacts, telephone calls, and collateral contacts, offering community, in-home, and clinic-based supports.
- Monitor progress and response to services, collaborating with individuals and their families to determine the need for changes in provision and array of services.
- Establish relationships with family members, natural supports, and care providers, as appropriate and with consent.
- Coordinate with medical providers, probation, DCS, and other agencies to ensure comprehensive care.
- Facilitate Child and Family Team meetings or Adult Recovery Team meetings for ongoing assessment and progress.
- Interact with all components of the behavioral health delivery system, primary care physicians, and other members of the child and family or recovery team.
- Obtain prior authorization for inpatient, partial hospitalization, residential, and other services that require it.
- Coordinate transportation as needed for medically necessary behavioral health or medical services.
- Document all interactions and services provided in progress notes in the electronic health record.
- Assist in identifying third-party liability.
- Coordinate all care and services to enhance and facilitate optimal physical and behavioral health wellness within a trauma-informed care framework.
- Bachelor's degree or master's degree in a field related to behavioral health or a health-related field.
- Bachelor's degree in any field.
- Associate's degree and two years of behavioral health work experience.
- High school diploma or high school equivalency diploma and four years of full-time behavioral health work experience.
- Demonstrated competency in Managed Care systems and provider networks.
- Demonstrated competency in the adult recovery model (for working with adults).
- Demonstrated competency in the philosophy of behavioral health programs.
- Experience or education in child development preferred for positions serving children.
- Demonstrated competency in case management roles and responsibilities.
- Demonstrated competency in case management procedures and allocation of resources.
- Demonstrated competency in the characteristics of behavioral health issues and levels of severity.
