Job Summary
We are seeking a skilled Social Worker to join our team at DCH Health System. As a key member of our interdisciplinary treatment team, you will play a vital role in coordinating patient care and providing comprehensive social work services to our patients and their families.
Responsibilities
- Develop and implement patient treatment plans, including education, discharge planning, and follow-up care.
- Conduct psychosocial assessments and provide individual and group therapy sessions to meet active treatment requirements.
- Participate in interdisciplinary treatment team meetings to coordinate patient care needs.
- Document patient progress and maintain accurate records.
- Collaborate with other healthcare professionals to ensure seamless patient care.
- Meet productivity and performance standards, as outlined in the performance evaluation.
- Perform other duties as assigned.
Requirements
- Master's degree in Social Work (MSW) or related field.
- LCSE (Licensed Certified Social Worker) certification preferred.
- Two years of inpatient hospital experience desirable.
- Effective communication and problem-solving skills.
- Ability to read, write, speak, and comprehend English.
Working Conditions
- Physical presence on-site is essential.
- Exposure to body fluids, disease, infection, lab chemicals, and hazardous materials.
- Must be able to perform duties with or without reasonable accommodation.
