Job Summary

We are seeking a highly skilled and dedicated Registered Nurse to join our team as a Home Health Registered Nurse. As a key member of our care team, you will be responsible for providing high-quality patient care and support to individuals in their homes. This is a full-time position that requires a strong commitment to delivering exceptional patient outcomes and exceeding productivity standards.

Key Responsibilities
  • Provide comprehensive patient care and support in the home setting, including assessment, planning, implementation, and evaluation of patient care plans.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans that meet the unique needs of each individual.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop personalized care plans to address these needs.
  • Deliver high-quality patient care and support, including wound care, medication management, and education on disease management and self-care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and transitions.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
  • RN license in the state of employment
  • Graduate from an approved school of nursing
  • One year of RN experience and clinical competence to deliver quality patient care
  • Current driver's license and ability to spend approximately 20% of work time driving to/from patient locations
  • Commitment to consistently meet critical deadlines for charting and documentation
  • Ability to self-manage time and schedule
  • Demonstrated experience with tablets, mobile phones, and EMR software
