Role Overview
The Neighborhood Center Physician is a healthcare professional responsible for diagnosing, managing, and caring for a range of health-related issues for patients at Suvida Healthcare. This role involves overseeing patients within a multidisciplinary value-based care framework, handling cases from low-risk to high-complexity, where thorough analysis of situations or data is essential. The physician will collaborate with a multidisciplinary care team and lead their respective care team within the Neighborhood Center.

Key Responsibilities
  • Provide care for Suvida Healthcare's patients, emphasizing outpatient medicine, continuity of care, health maintenance, improving health outcomes, and disease prevention.
  • Manage patients within a multidisciplinary value-based care model.
  • Lead the respective care team within the Neighborhood Center.
  • Collaborate effectively with the multidisciplinary care team at Suvida Healthcare.
  • Focus on delivering 5-Star care and maintaining a high level of customer service.
  • Maintain accurate medical histories and records.
  • Refer patients to specialists as necessary.
  • Participate in daily morning huddles with the care team to discuss patients scheduled for the day and address any potential care gaps.
  • Engage in weekly huddles with the multidisciplinary care team to review and manage high-risk patients.
  • Contribute to shaping departmental strategy.
  • Make decisions on moderately complex to complex issues regarding technical approaches for project components, working independently.
  • Exercise considerable latitude in determining objectives and approaches to assignments.
  • Oversee Advanced Practice Providers as needed within the Neighborhood Center.
  • Report to the Neighborhood Center Medical Director.

Qualifications
  • Minimum of 3 years of experience, preferably in geriatrics.
  • Active, unrestricted medical license in Texas or Arizona.
  • Current on all required immunizations.
  • Understanding and support of a managed care, full risk, value-based care model preferred.
  • Strong leadership, customer service, and communication skills, both spoken and written.
  • Ability to demonstrate self-confidence and a positive attitude.
  • Excellent team player with strong communication skills.
  • Proficiency in Microsoft Office applications including Word, Excel, and PowerPoint.
  • Bilingual/Bicultural (English and Spanish) preferred.

Education and Certification Requirements
  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) degree.
  • Board certification or eligibility in Family or Internal Medicine.
  • Current DEA license.
