Hospice Care Opportunity

We are seeking a dedicated and compassionate Hospice Care Assistant to join our team at LifeBridge Health. As a key member of our care team, you will provide personalized support and care to patients in need, working closely with our Registered Nurses to ensure the highest level of care.

Responsibilities:
  • Deliver high-quality patient care and support services in accordance with our Mission Statement and S.P.I.R.I.T. Values.
  • Work collaboratively with our healthcare team to develop and implement personalized care plans.
  • Provide emotional support and companionship to patients and their families.
  • Assist with daily living activities, such as bathing, dressing, and grooming.
  • Monitor patient vital signs and report any changes to the healthcare team.
  • Participate in ongoing education and training to enhance your skills and knowledge.
Requirements:
  • High School Diploma or equivalent required.
  • Successful completion of Certified Nursing Assistant course required.
  • Previous experience as a CNA/Home Health Aide, preferably in a Medicare Certified Program, preferred.
  • CPR certification required.
  • Ability to communicate effectively with patients and families.
  • Reliable transportation and flexibility to adapt to changing schedules and assignments.
