Job Summary:

Juliette Fowler Homes is seeking a compassionate and detail-oriented Senior Living Coordinator to join our team. As a key member of our community, you will play a vital role in ensuring the well-being and independence of our residents.

Key Responsibilities:
  • Maintain awareness of residents' physical and mental conditions, providing oversight and support as needed.
  • Develop and maintain a network of service providers and local community resources, ensuring residents have access to necessary support services.
  • Provide general case management for residents, making referrals to service providers and community agencies as required.
  • Identify and assess residents at risk, recognizing when they need a higher level of care to maintain or improve their safety and health.
  • Document all incidents related to residents' conditions or situations, maintaining accurate and up-to-date records.
  • Respond to resident emergency calls during regular office hours, providing timely and effective support.
  • Conduct annual apartment inspections with maintenance, assessing residents' living environments and making recommendations for improvement.
  • Plan, schedule, and perform activities for residents, providing social opportunities that foster informal support networks.
  • Act as an ex-officio member of the resident council, attending meetings and accepting reports from wing-representatives and monitors.
  • Visit with and follow up on residents in the hospital, coordinating hospital reports with staff, monitors, and wing-representatives.
  • Respond to inquiries concerning eligibility for HUD and HUD/STAR+ paired Assisted Living facilities, maintaining current knowledge of relevant regulations.
  • Provide education and training on service availability, application procedures, residents' rights, aging in place, and other topics as appropriate to staff, families, and the community.
  • Organize, implement, and facilitate residents' volunteer activities within the community, coordinating and assisting with outside volunteer activities and groups.
Qualifications/Requirements:
  • Graduate of an accredited university with a degree in Social Work, Gerontology, Psychology, Sociology, or Counseling.
  • Verifiable experience in a social service delivery system.
  • Experience working with the elderly and/or disabled, with knowledge of the aging process and related issues.
  • Experience in the successful development and execution of a senior activity program.
  • Strong interpersonal skills, with the capacity to relate to a variety of people and personalities.
  • Strong analytic skills, with the ability to form comprehensive solutions to analyzed situations.
  • Strong organizational, record-keeping, and time-management skills, with the ability to work independently and as part of a team.
  • Working knowledge of personal computers and ability to learn various programs/applications.
  • Valid driver's license and ability to operate a motor vehicle.
  • Ability to work flexible hours and be on call.
  • Ability to comply with residents' rights, all regulatory requirements, and all company policies and procedures.
  • Ability to read, write, speak, and understand the English language.
  • Ability to make independent decisions with circumstances warranting such action.
  • Supportive team member, contributing to and being an example of teamwork and team concept.
  • Leadership ability and willingness to work harmoniously with other personnel.
  • Patient, tactful, cheerful disposition, and enthusiasm, with the ability to handle residents based on their current maturity level.
  • Bilingualism is a plus.
