Job Summary:The successful candidate will be responsible for assessing and evaluating the needs of patients with multiple disease processes and comorbidities, developing and implementing individualized care plans, and providing education and strategies for patients and families.
Responsibilities:- Participate in a comprehensive clinical orientation program to learn key procedures, the electronic medical record system, and compassionate care principles.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
- Collaborate with other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
- Document all changes in patients' conditions, updating care plans and outcomes as necessary.
- RN with a Bachelor's degree in Nursing, preferred.
- At least one year of supervised clinical experience as a registered nurse.
- Excellent verbal and written communication skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong computer skills, with proficiency in electronic medical records and other healthcare software.
- Excellent organizational and interpersonal skills, with the ability to work independently and as part of a team.
