We are currently seeking a dedicated Practical Nursing Specialist to deliver exceptional care to our residents.

Key Responsibilities:

  • Administers nursing treatments and personal care services to residents, adhering to the scope of practice for LPNs, which includes the prevention and management of pressure ulcers, monitoring dietary intake, ensuring patient hygiene, preparing injections/enemas, taking vital signs, and assisting with feeding and dressing changes.
  • Responsible for the accurate administration of medications, including the medication pass, ordering, and maintaining controlled substances, while observing and reporting any adverse reactions.
  • Coordinates diagnostic testing as prescribed and communicates results to the appropriate nursing staff or physician in accordance with facility protocols.
  • Maintains comprehensive medical and nursing records for residents, documenting vital signs, weight, and fluid/nutritional intake and output.
  • Collects samples for laboratory testing and performs routine tests as directed.
  • Monitors intravenous therapy, executes prescribed treatments, and dispenses medications following established policies.
  • Documents and reports all incidents involving residents and staff to the nursing supervisor and implements prescribed medical and nursing procedures.
  • Operates specialized equipment and provides guidance to the nursing team as necessary.
  • Assists new residents in acclimating to the facility and educates them on rules and regulations.
  • Prepares residents and their families for potential discharge.
  • Ensures compliance with fire and safety regulations to maintain a secure environment for residents.
  • Facilitates nursing instructions for Certified Nursing Assistants (CNAs) to promote patient safety and quality care.
  • Upholds infection control standards, including universal precautions and hand hygiene protocols.
  • Identifies residents at risk for elopement and injury, implementing safety measures accordingly.
  • Provides thorough reports to incoming nursing staff regarding resident conditions and any unusual occurrences.
  • Demonstrates sensitivity to the unique needs of residents with dementia and those who are subacute or terminally ill, completing required documentation daily.
  • Adheres to the code of conduct, ensuring resident privacy and compliance with HIPAA regulations.
  • Performs additional related duties as assigned.
Qualifications, Skills, and Abilities:

Education:
Completion of a formal nursing education program.

License/Certification:
Valid NYS LPN License, CPR certification required.

Technical Skills:
Proficient in basic computer and EMR software.

Physical and Mental Demands:

Ability to walk long distances, climb stairs, and remain on feet for extended periods. Must be able to bend, squat, push, pull, reach, stretch, and lift up to 50 lbs. Proficient in spoken and written English, with good hearing capabilities. Must demonstrate hand-eye coordination for operating various medical equipment.
