Job Overview:
We are seeking a compassionate Nurse Practitioner (FNP/AGNP) to provide evidence-based medical care to seniors and patients with complex needs. As a House Calls Nurse Practitioner, you will work independently, assessing patient needs, developing treatment plans, and coordinating care with healthcare professionals.

Responsibilities:
  • Perform comprehensive health assessments and develop individualized care plans
  • Order and interpret laboratory and diagnostic testing as needed
  • Collaborate with caregivers and other healthcare professionals to ensure coordinated care
  • Provide referrals to specialists when necessary

Requirements:
  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Master's degree in Nursing (MSN)
  • At least two years of nurse practitioner experience
  • Strong communication and interpersonal skills
