As a key member of our Infectious Diseases Clinic, you will play a vital role in providing guidance and assistance to patients in assessing medical, insurance, housing, social services, community, legal, financial, and other services.

Responsibilities:
  • Provide guidance and assistance to patients in assessing medical, insurance, housing, social services, community, legal, financial, and other services.
  • Support the medical case management team with completion of intakes, psychosocial assessments, development and re-evaluation of individualized client service plans, and follow up on referrals as needed.
  • Perform client advocacy and patient retention activities, treatment adherence counseling, and termination or inactivation of cases.
  • Maintain documentation in the EPIC, NJDOH system (CareWare), and Ryan White Grant reporting systems (CHAMPS), ensuring accurate and prompt data entry of service deliverables.
  • Educate and consult with service providers and consumers concerning HIV disease and the process of meeting consumers' identified needs.
Qualifications:
  • Minimum of one year's experience working in case management, discharge planning, or other health-related fields, preferably with persons with or at high risk of HIV infection.
  • High School degree required; Associate degree or higher in Social Work, Psychology, or other healthcare-related fields preferred.
  • Ryan White Case Management Certificate obtained within 6 months of hire is required.
  • Experience and knowledge in basic to intermediate computer skills (Microsoft Office: Word, Excel, PowerPoint, email, etc.).
