Job Description:
Your Role: As a Registered Nurse, you will be a vital member of our PACE program team, providing high-quality, patient-centered care to older adults. You will work collaboratively with healthcare professionals to develop and implement personalized care plans, addressing complex needs and promoting overall well-being.

Main Responsibilities:
  • Conduct thorough patient assessments, identifying individual needs and developing targeted care plans.
  • Collaborate with healthcare providers to deliver evidence-based treatments and ensure seamless care coordination.
  • Monitor patient progress, adjusting care plans as needed to optimize outcomes.
  • Participate in quality improvement initiatives, contributing to best practices and service excellence.
  • Engage with patients and families through empathetic communication, education, and advocacy.

Qualifications:
  • Degree in nursing from an accredited institution with active RN licensure.
  • At least 3 years of clinical experience, preferably in geriatric care or a related field.
  • Proficient in electronic medical records and paperless documentation.
  • Strong interpersonal skills, with ability to build rapport with patients, families, and colleagues.
  • Basic computer literacy and willingness to learn specialized software applications.
  • A valid driver's license and reliable transportation are required.
  • Background clearance, including criminal history and substance abuse screening, is mandatory.
