About the Role:We are seeking an experienced Physical Therapist to provide functional rehabilitation services in an outpatient setting. As a member of our collaborative and passionate team, you will have the opportunity to make a meaningful difference in the lives of our clients.
Responsibilities:- Perform evaluations and develop treatment plans to address the unique needs of each client.
- Regularly re-assess the effectiveness of treatment plans and communicate with the treatment team, physician, and family members to ensure seamless care.
- Provide discharge evaluations and make recommendations for home and community safety, as well as provide referral and education resources.
- Communicate effectively with other disciplines to ensure collaboration, coordination of care, and enhance patient outcomes.
- Minimum of bachelor's degree in Physical Therapy from an accredited program.
- Current, unrestricted license as a Physical Therapist in the state where practicing.
- Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations.
- A minimum of one year's work experience as a physical therapist preferred.
- Demonstrates knowledge of rehabilitation techniques related to complex neurological injury.
- Communicates effectively and professionally in verbal and written interactions.
- Ability to lift 50 pounds and perform duties that require fine motor skills, visual acuity, and walking/standing for extended periods.
