We are seeking a compassionate Physical Therapist to provide medical support to our clients in their homes.

Job Summary

The ideal candidate has a can-do attitude, excellent interpersonal skills, and the ability to provide high-quality patient care.

Responsibilities
  • Visit patients in their homes for one-on-one visits to assess their physical, psychosocial, and environmental needs.
  • Chart observations, perform evaluation tasks, and administer medication as prescribed by the patient's physician.
  • Educate patients and their families on proper home health care strategies and procedures.
  • Provide effective communication to patients/clients, their family members, team members, and other healthcare professionals.
Qualifications
  • Graduate of an accredited Physical Therapy Program.
  • Current state license as a Physical Therapist.
  • Proof of current CPR certification.
  • Compassionate, friendly demeanor.
  • Excellent communication skills.
  • Current driver's license and reliable transportation.
  • Ability to work a flexible schedule and travel locally.
