Job Summary

We are seeking a highly skilled and compassionate Team Manager to lead our Patient Care Team. As a key member of our interdisciplinary team, you will be responsible for supervising, evaluating, and coordinating the care of our patients and their families.

Key Responsibilities
  • Supervise and evaluate team members to ensure high-quality patient care
  • Coordinate patient care services, including volunteer and bereavement services
  • Review patient information to ensure eligibility and appropriateness criteria are met
  • Collaborate with physicians and other healthcare providers to ensure timely and effective care
  • Monitor and improve team performance, including productivity and labor costs
  • Develop and implement quality improvement initiatives to enhance patient care and satisfaction
  • Provide exceptional customer service and resolve patient and family concerns
  • Participate in professional development and education to stay current with industry best practices
Requirements
  • 2+ years of management experience
  • 3+ years of RN experience
  • Strong leadership and communication skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
  • Knowledge of hospice care and regulatory requirements
