Job Title: Infusion Nurse Practitioner

AleraCare is seeking a skilled Infusion Nurse Practitioner to join our team at our Mountain View, CA infusion center. As a key member of our healthcare team, you will provide high-quality patient care and contribute to the success of our organization.

Responsibilities:
  • Provide comprehensive nursing services to assigned patients, including initial evaluations, assessments, and development of individualized plans of care.
  • Collaborate with physicians and other healthcare professionals to ensure optimal patient outcomes.
  • Conduct regular reassessments and updates to patient plans of care as needed.
  • Order and interpret lab work and diagnostic tests, and prescribe medications and non-pharmacological therapies as appropriate.
  • Participate in the company's Performance Improvement and Continuous Quality Improvement (CQI) programs.
  • Assume responsibility for clinic location(s) and support a culture of safety and quality care.
Requirements:
  • Master's Degree from an accredited Nurse Practitioner Program.
  • Current NP state license in state of practice.
  • At least 4-5 years of advanced practice registered nursing (APRN) experience, with ambulatory and infusion nursing experience preferred.
  • Strong interpersonal, time management, and organizational skills.
  • Strong written and verbal communication skills.
  • Experience with electronic medical records (EMR) and computer skills, including word processing and efficient use of the internet and e-mail.
