Infusion Nurse Practitioner

AleraCare is seeking a skilled Infusion Nurse Practitioner to join our team at our Mountain View, CA infusion center. As a key member of our healthcare team, you will provide high-quality patient care and contribute to the success of our organization.
Job SummaryThe Infusion Nurse Practitioner will be responsible for providing nursing services to assigned patients in accordance with their plan of care. This includes conducting initial patient evaluations, developing and implementing care plans, and collaborating with healthcare teams to ensure optimal patient outcomes.
Responsibilities- Provide patient-centered care, maintaining confidentiality and treating patients and families with respect and dignity.
- Conduct initial patient evaluations and develop written plans of care, ensuring accurate documentation and communication with healthcare teams.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care.
- Order and interpret lab work and diagnostic tests as needed, and prescribe medications and non-pharmacological therapies based on patient diagnoses and health history.
- Participate in Performance Improvement and Continuous Quality Improvement (CQI) programs, and contribute to a culture of safety and quality care.
- Stay current with medical advancements, new technology, and new drugs through ongoing education and professional development.
- Master's Degree from an accredited Nurse Practitioner Program.
- Holds a current NP state license in the state of practice.
- At least 4-5 years of advanced practice registered nursing (APRN) experience, with ambulatory and infusion nursing experience preferred.
- Strong interpersonal, time management, and organizational skills, with excellent written and verbal communication skills.
- Experience with electronic medical records (EMR) and ability to maintain accurate patient care records.
- Current CPR card, advanced practice certification, current APRN licensure, valid driver's license, reliable transportation, and automobile insurance.
