About the Role

We are seeking a skilled and compassionate Advanced Practice Clinician to join our Housecalls team as a Traveling Healthcare Professional. As a key member of our team, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes.

Key Responsibilities
  • Conduct comprehensive in-home assessments, including medical history reviews, medication reconciliation, and physical exams.
  • Perform evidence-based practice screenings and point-of-care testing as needed.
  • Develop and implement personalized care plans to address patients' unique needs and health goals.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Travel up to 85% of the time to visit patients in their homes, requiring a valid driver's license and reliable transportation.
Requirements
  • Active NP or PA license in good standing, with the ability to obtain additional licensure as needed.
  • Current national certification in NP or PA, with a focus on family, gerontology, internal medicine, or endocrinology.
  • 1+ years of clinical experience in a healthcare setting, with a strong foundation in gerontology, cardiology, or internal medicine.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
