Transforming Care Delivery with Optum HouseCalls

At Optum, we're revolutionizing healthcare by bringing care to patients in the comfort of their own homes. As a Nurse Practitioner (NP) or Physician Assistant (PA) on our HouseCalls team, you'll play a vital role in delivering high-quality, patient-centered care.

Key Responsibilities:

  • Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing (as appropriate).
  • Identify diagnoses to inform care management and active medical management in the furtherance of treatment.
  • Communicate findings to inform members' primary care physicians of potential gaps in care.
  • Intervene in urgent and emergent situations, and educate members on disease processes, medication, and compliance.

Requirements:

  • Active NP or PA license, or ability to obtain by start date.
  • Licensure must be unencumbered, free of any open/unresolved disciplinary actions.
  • Active national NP or PA certification, or ability to obtain national certification in the state of assignment.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag, navigating stairs, and sit, stand, and kneel to perform physical assessments.
  • Reliable transportation for home visit assessments.

Preferred Qualifications:

  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
  • Ability to travel up to 5% within licensed states for additional incentives.
