At Optum, we're revolutionizing healthcare by bringing care to patients in the comfort of their own homes. As a HouseCalls Statewide Traveler, you'll play a vital role in delivering high-quality care and improving health outcomes for our patients.

Key Responsibilities:
  • Conduct in-home assessments, including medical history reviews, medication reconciliations, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing
  • Identify diagnoses and develop care management plans
  • Communicate findings with patients' primary care physicians to ensure seamless care coordination
  • Travel up to 85% of the time, exploring new destinations and experiences
Requirements:
  • Active NP or PA license or ability to obtain by start date
  • Active national NP or PA certification or ability to obtain national certification in state of assignment
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions
Preferred Qualifications:
  • 1+ years of clinical experience in a relevant setting
  • Experience in gerontology, cardiology, internal medicine, or endocrinology
  • Home health care or home visit experience
