Mental Health Cooperative is seeking a skilled Behavioral Health Care Manager to join our team. As a Care Manager for Adults, you will play a critical role in providing comprehensive care management services to consumers with severe and persistent mental illness (SPMI) and their supports.
Key Responsibilities- Conduct continual assessments of consumers' strengths, needs, supports, and goals.
- Meet with consumers and their supports in the community to provide support and guidance.
- Collaborate with team members, agency employees, and community partners to promote consumer success in accessing care and making progress towards goals.
- Develop and implement comprehensive and individualized care plans, crisis plans, and other necessary documents.
- Provide education to consumers and their supports regarding proper usage of emergency services, primary care physicians, and other healthcare resources.
- Ensure timely and appropriate documentation as required by the agency.
- Bachelor's Degree in a health-related field, such as counseling, psychology, social work, or other behavioral health field.
- Valid Tennessee Driver's License and acceptable Motor Vehicle Report (MVR).
- Acceptable Criminal Background Investigation and Personal Automobile Insurance.
- Cell Phone and Transportation That Seats 4 People.
- 'F' Endorsement.
